On behalf of our client in public health, we are seeking a motivated and compassionate Social Worker to join the multidisciplinary team within a Special Care setting, supporting young people with complex needs and high-risk behaviours. The successful candidate will work collaboratively with young people, families, residential staff, external agencies, and healthcare professionals to promote positive outcomes, safeguard welfare, and support care planning and intervention strategies.
Key Responsibilities
Undertake comprehensive social work assessments of young people referred to or placed within Special Care services.
Contribute to the development, implementation, and review of individual care plans and therapeutic interventions.
Work directly with young people and their families to support engagement, family relationships, and transition planning.
Participate in admission, care planning, and discharge processes for young people within the service.
