On behalf of our client in public health, we are seeking a motivated and compassionate Social Worker to join the Safety & Welfare Team based in Louth . The successful candidate will work as part of a multidisciplinary team, providing professional social work services to individuals and families, promoting safety, wellbeing, and positive outcomes through assessment, intervention, advocacy, and collaborative care planning.
Key Responsibilities
Undertake comprehensive psychosocial assessments to identify individual and family needs, risks, and appropriate interventions.
Develop, implement, and review person-centred care and support plans in collaboration with service users, families, carers, and multidisciplinary colleagues.
