This is an excellent opportunity to support patients and families through assessment, discharge planning, safeguarding, crisis intervention, and coordination of care within a fast-paced healthcare environment.

Key Responsibilities

• Provide psychosocial assessments and interventions
• Support patients and families during admission and discharge planning
• Work collaboratively with multidisciplinary healthcare teams
• Participate in safeguarding and protection procedures
• Maintain accurate records and professional standards

Requirements

• Degree or Masters in Social Work
• CORU registration or eligibility for registration
• Experience in healthcare, hospital, or public sector settings desirable
• Strong communication, teamwork, and case management skills
