Job Description

AllPro Recruitment are seeking a Social Care Worker to join a Primary Care Social Work Team, providing support to individuals living in the community and helping them to live independently and achieve their personal goals.

This is a rewarding opportunity for a qualified Social Care professional who is confident working directly with clients and as part of a wider Social Work and multidisciplinary team.

Key Responsibilities

  • Provide person-centred social care support based on individual needs and personal plans.
  • Build positive relationships with clients and promote their independence, dignity and wellbeing.
  • Support clients with daily living, including budgeting, hygiene, household tasks and personal organisation.
  • Implement and evaluate care plans and interventions under the direction of the Social Work Team.
  • Support clients to attend medical, clinical, training and community appointments.
  • Act as a keyworker and coordinate formal and informal support networks.
  • Participate in care planning, case reviews and multidisciplinary meetings.
  • Liaise with statutory and voluntary agencies to arrange appropriate services.
  • Advocate for clients' rights, choices and needs.
  • Provide accurate written and verbal updates on client progress.
  • Assist with referrals, transfers and case closures where appropriate.
  • Maintain professional documentation and confidentiality in line with HSE and CORU requirements.
  • Participate in supervision, training, professional development and quality improvement.

Requirements

  • A recognised qualification relevant to Social Care / Social Work.
  • Demonstrable professional knowledge and experience relevant to the Social Care Worker role.
  • Knowledge of appropriate social care interventions and approaches.
  • Experience supporting individuals in a person-centred environment.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organisational and time-management skills.
  • Ability to manage competing priorities and work effectively in a busy environment.
  • Good computer skills, including Microsoft Word, Excel, email and internet use.
  • Strong understanding of professional boundaries, confidentiality, safeguarding and risk management.
  • A commitment to ongoing professional development.
  • Ability to advocate effectively for clients and promote their access to appropriate supports and services.
