On behalf of our client within the healthcare sector, we are currently seeking experienced Social Care Workers to join a supportive care team in a new location setting. These roles are offered on long-term temporary contracts, with both full-time and part-time positions available.
Main Duties & Responsibilities
Contribute to the development, implementation, and review of individual care plans, ensuring all records are accurately maintained.
Promote and uphold the rights, dignity, privacy, and independence of individuals in care.
Provide support for individuals’ physical, emotional, social, and personal wellbeing.
Encourage and assist individuals to participate in appointments, education, training, and community-based activities.
Support individuals with day-to-day administrative tasks and personal documentation where required.
